Questions to ask while FORMing
- What makes you interested?
- What are your fitness and nutrition goals?
- Why are those your goals?
- When would you like to achieve these goals by?
- Where do you struggle most, with fitness or nutrition?
- What kind of workouts do you like? What kind of workouts do you not like?
- What kind of equipment do you have? (resistance bands, weights, etc)
- How soon would you like to start?
- Have you attempted anything recently that did not work or is there anything you know you hate?
- When/ if you used to be active/fit on a regular basis, what were some of your favorite activities / what do you like?
- How active are you now and do you have an physical limitations / injuries I need to be aware of?
- How long (days / months / years) are you willing to work on this goal and how much time to you have each day?
- How would you describe your bad eating habits / meal planning / tracking? (i.e. too picky, get lazy with food, default to fast food, love sweets etc.)
- Do you currently own any in-home workout programs? Which ones?
- How will you feel when you accomplish your goals?
